1. Purpose of this Policy

At Angulo & Associates LLC, we respect and protect the privacy of our clients. This Privacy Policy explains what personally identifiable information (PII) we collect, why we collect it, how we use/share/protect it, and how you can exercise your rights. This policy applies to all clients, members, and applicants who receive assistance through our agency.

2. Information We Collect

We may collect the minimum necessary PII to assist you, including: name, date of birth, Social Security Number, address, phone, email, immigration/citizenship data (when required), household size, income and tax information, medical/health-related eligibility details, and documents needed for eligibility and enrollment.

3. How We Use Your Information

We use your PII only for authorized purposes: helping determine eligibility for Virginia’s Insurance Marketplace, Medicaid, or FAMIS; applying for APTCs/CSRs; plan selection, enrollment, renewals; required notices; permitted referrals (e.g., tax/legal aid); and authorized follow-ups or educational information. We do not use or share PII for unrelated marketing or sales.

4. Sharing Your Information

We may share PII only with: Virginia’s Insurance Marketplace and applicable government agencies; health insurance carriers to complete enrollment; licensed professionals or organizations you are referred to (with your consent); and authorized, trained employees. We never sell your PII or share with unauthorized third parties.

5. Consumer Consent

Before collecting or using your PII, we explain what we collect, why, and how it will be used; provide this Privacy Policy; and obtain your informed consent (written, verbal, or electronic). You may revoke or limit consent at any time. Revocation may affect our ability to assist with enrollment or servicing.

6. Safeguards and Protection

We maintain reasonable safeguards to protect PII: locked storage for paper records; password-protected and, where applicable, encrypted electronic systems; role-based access; secure transmission practices; and secure destruction when retention periods end (e.g., shredding, wiping). Employees receive training in privacy, HIPAA, and fraud prevention.

7. Retention of Records

We retain consent and enrollment records for at least six (6) years, or longer if required by law.

8. Your Rights

You have the right to know what PII we collect and how it is used, to request corrections, and to file a privacy complaint with us or directly with Virginia’s Insurance Marketplace.

9. Privacy Incidents and Breaches

If PII is lost, accessed without authorization, or improperly disclosed, we will act to contain the issue, report to Virginia’s Insurance Marketplace within 24 hours as required, and notify you if your information was compromised.

10. Fraud Prevention

We will never ask you to sign blank forms, or share banking details outside secure channels.
